Provides primary care services including mental health and addiction medicine. Does not provide maternity care, and does not see children.
Columbia Street Community Clinic has two physicians on staff each day, from a team of 13 family doctors and addiction medicine specialists providing both in-person and telehealth services on a drop-in basis, five days a week.

It provides access to low-barrier primary care services, mental health and addiction treatment services. These include:
  • Assessment and same-day prescriptions for methadone, suboxone and various opioid agonist treatment (OAT) options including fentanyl patches and fentanyl powder.
  • Access to lab work, immunizations, monthly Hep C and women’s health clinics.
  • Access to various other in-house specialist clinics, including Internal Medicine and Infectious Disease.

Referral Instructions: Patients who are not attached to care elsewhere may self-refer using the form below or on a walk-in basis for both primary and addiction care. Patients with primary care elsewhere require a referral from their physician to access addiction medicine programs.
