L2T – Linked to Treatment Program
Condition Specific Support and Public Health
Provided by Nanaimo and Area Resource Services for Families (NARSF)
The program uses evidence-based interventions to promote and monitor treatment adherence. As treatment regimens are complex and the impact on health and wellness significant, L2T welcomes community partners’ involvement in a holistic, connected care team approach and in cases where there are physician referrals and coordination support for the referred person.
Program Eligibility
- Newly diagnosed with HIV and/or HCV
- Re-engagement into care for HIV and/or HCV positive individuals
- HIV and/or HCV positive individuals who would benefit from strengthening their engagement into care
- Able to provide legal consent >16yrs
Duration of Program: 3-9 months, Intensive case management support.
