Older Adult Mental Health Program

Provided by Vancouver Coastal Health

Program offers specialized services for adults 65 years and older with mental illness, substance use, dementia, behavior issues related to dementia, and physical and functional decline
The Older Adult Substance Use Program team consists of five counsellors and one Addictions Medicine Physician. The team works within a client-centered, trauma-informed, harm-reduction framework using a bio-psycho-social-environmental-spiritual model. Clients are seen throughout the city of Vancouver.

The counsellors meet clients in their homes (this includes care facilities and shelters), in office, by phone, and virtually. Services provided include assessment, counselling, referral, case management that supports client recovery, support groups, and psycho-educational groups.

Program provides:
  • Specialized assessment and consultation
  • Facility liaison consultation
  • Treatment/case management
  • Education

The rehabilitation program offers:
  • Individual occupational therapy assessments
  • Psychosocial day program
  • Peer support, and group programs

Eligibility:
  • A resident of Vancouver
  • Aged 65+
  • Wanting to make changes to their substance use
  • Agreeable to seeing a counsellor related to their problematic substance use (which may include alcohol, drugs or prescription medication).

Please connect with a/your family physician for a referral. They simply need to approve of your request, or referrals from the community and facilities.
