Older Adult Mental Health Program
Condition Specific Support, Mental Health - Senior, and Services for Seniors
Provided by Vancouver Coastal Health
The counsellors meet clients in their homes (this includes care facilities and shelters), in office, by phone, and virtually. Services provided include assessment, counselling, referral, case management that supports client recovery, support groups, and psycho-educational groups.
Program provides:
- Specialized assessment and consultation
- Facility liaison consultation
- Treatment/case management
- Education
The rehabilitation program offers:
- Individual occupational therapy assessments
- Psychosocial day program
- Peer support, and group programs
Eligibility:
- A resident of Vancouver
- Aged 65+
- Wanting to make changes to their substance use
- Agreeable to seeing a counsellor related to their problematic substance use (which may include alcohol, drugs or prescription medication).
Please connect with a/your family physician for a referral. They simply need to approve of your request, or referrals from the community and facilities.
