Provides support for seniors to access non-clinical, community-based services.
The Social Prescribing Program supports seniors 65+ to access non-clinical, community-based services to
prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based
approach. Social Prescribing is an approach to offering non-medical support that enables older adults to identify, set and achieve personal health and wellness goals. A Community Connector helps to find appropriate local
programs and services and provides regular check-ins to ensure clients are on track. This program is intended for adults 65+ living in the Westshore (Langford, Colwood, Sooke, Metchosin) who meet two or more United Way BC Healthy Aging priority criteria, such as:
- Low to modest income
- Low to moderate frailty
- Social isolation or loneliness
- Member of an underserved or equity-deserving group
