Seniors’ Community Connector - Social Prescribing Program - Victoria (Oak Bay, Victoria, Esquimalt, View Royal, Saanich))

The Social Prescribing Program supports seniors to access non-clinical, community-based services to prevent or delay frailty by fostering resilience and social support using a comprehensive, strength-based approach.
The Oak Bay Volunteer Services Community Connector program takes a social prescribing approach to enhance the health and well-being of adults aged 55 and over in Greater Victoria, BC. By connecting older adults to non-medical, community-based services and activities, the program addresses social isolation, frailty, and other factors affecting health. This holistic, person-centered model supports seniors in improving their overall quality of life through tailored community engagement.

Eligibility: Any older adult 55+ residing in Victoria (residing in LHA 411 and CHSA 4131), who is either experiencing or is at risk of:
  • Frailty
  • Social Isolation and/or Loneliness
  • Barriers to Accessing Community-Based Resources
  • Frequent Trips to Emergency Department or Primary Care
  • Experiencing Life-Limiting Illness (Congestive Heart Failure, Renal Failure, COPD, Diabetes, etc.), End-of-Life, or Palliative Care
