A dedicated team that will come in to the home to administer medication.
Heart at Home offers in home medication administration to Heart Pharmacy clients with a dedicated group of Licensed Practical Nurses, Registered Nurses, and Home Care Aides. Clients are offered daily assistance with:
- Taking daily medications
- Eyedrop and Eardrop Administration
- Medicated Patch Application
- Insulin Injections and Blood Glucose Monitoring
- Blood Pressure Monitoring
- Inhaler Administration
- Daily wellness check in
- Companionship
- Help navigate applications for "Special authority" medications (medications that are benefits under specific circumstances)
For patients taking three or more medications, the medication assistance program is typically covered through provincial or personal health plans, subject to the patients’ deductible. The program costs $30 per day, plus the cost of medication, if there is no coverage. Additionally, certain groups, such as First Nations individuals and psychiatric patients, receive full medication coverage regardless of income.
