Intensive Case Management for Youth - Mobile Mental Health and Addictions Support - Island Health

Provides integrated service to youth who have multiple and complex needs related to persistent and significant substance use and co-occurring mental health challenges.
The Youth Intensive Case Management Team (YICMT) is for youth (primarily 13-18 but up to age 25) in community experiencing the most persistent and complex substance use with additional vulnerabilities. These might include:
  • homelessness and precarious housing
  • living as members of a marginalized population
  • living with mental health challenges and/or developmental disabilities
  • being high risk for exploitation
  • experiencing heavy criminal justice involvement


The Team utilizes a wraparound team case management approach offering both brokerage and direct service, to provide personalized care unique to each youth. The goals of the team are to improve health, social functioning, and access to care by offering a predominately intensive outreach approach providing comprehensive assessment, care planning, consultation, case management and systems navigation.

Youth who are supported by the YICMT access services in the community as opposed to office-based settings, reducing barriers and improving accessibility to care. YICMT is not a youth crisis team. It has a caseload of clients who require case management services. Core services include:
  • Community collaboration including referrals, liaising and advocacy
  • Case management and outreach
  • Crisis assessment, planning and intervention
  • Support to youth’s family and community
  • Individual, group and family skill building
  • Life skills support, including school and employment
  • Medication management
  • Harm Reduction supports
  • Collaboration with Psychiatry
